Supervised
Long-Term Exercise Program
The Supervised Long-Term Exercise (SLTE) program
is for the injured worker who has completed the appropriate course
of physical therapy yet requires a continuing exercise program to
either maintain functional status or achieve additional gains.
The SLTE program is a cost-effective alternative
for workers comp clients who require ongoing monitoring and supervision
yet require only periodic skilled intervention by a physical therapist.
San Joaquin’s SLTE program includes the following:
• Initial evaluation by a physical therapist
(this step is not necessary if client is a current SJVR patient).
• Development of an exercise program by a physical therapist.
• Ongoing supervision by an exercise physiologist.
• Subjective assessment by the exercise physiologist of the
client’s progress at each session.
• Bi-weekly conference by the exercise physiologist and physical
therapist to review the client’s program and progress.
• Monthly report (at every other conference) to the physician
and case manager.
Prescriptions for the SLTE must be specific as
to the frequency and duration. Minimum requirements for participation
in the SLTE are two-three visits per week for four weeks.
